entity and/or web-broker.*Effective Plan YearThe consecutive 12 month period during which a
health plan provides coverage for health benefits,
described at 45 CFR 155.20. A plan year may be a
calendar year or otherwise.MarketplacesThe Marketplaces for which the agent or broker
has completed FFM Registration. 'Individual'
indicates that the agent or broker has completed
FFM Registration for the Individual Marketplace;
'SHOP' indicates that the agent or broker
has completed FFM Registration for the SHOP
Marketplace; 'Both' indicates that the agent or
broker has completed FFM Registration for both
Marketplaces. Consistent with 45 CFR 155.220(l),
the FFM registration requirements also extends to
agents and brokers participating in State-based
Marketplaces on the Federal Platform.Registration Completion DateThe date on which the agent or broker completed
all applicable registration requirements (e.g.,
training, testing, signing the Marketplace
Agreement(s), and identity proofing) to
participate in a Federally-facilitated Marketplace
or State-based Marketplace on the Federal Platform
for the applicable plan year. If the agent or
broker is participating in both Marketplaces
(Individual and SHOP), but completed the
registration requirements for each Marketplace
on two different dates, this date represents the
earlier of the two dates.Individual Registration DateThe date on which the agent or broker completed
Individual Marketplace registration requirements
(i.e., training, testing, signing the Marketplace
Agreement and identity proofing) to participate in
the Individual Marketplace for the applicable plan
year.SHOP Registration DateThe date on which the agent or broker completed
SHOP registration requirements (i.e., signing
the Marketplace Agreement and identity proofing)
to participate in the SHOP Marketplace for the
applicable plan year.Termination or Suspension DateThe effective date of the termination or
suspension of the agent's or broker's Individual
and/or Small Business Health Options Program
(SHOP) Marketplace registration and Marketplace
agreement(s). During the suspension period and
upon termination, the agent or broker is not
permitted to assist with or facilitate enrollment
of qualified individuals, qualified employers, or
qualified employees in coverage in a manner that
constitutes enrollment through the Marketplaces,
or permitted to assist individuals in applying
for advance payments of the premium tax credit and
cost-sharing reductions for qualified health plans
(QHPs) offered through the Marketplaces. The agent
or broker must continue to protect any personally
identifiable information accessed during the term
of its Marketplace agreement(s). Termination and
suspension are described in regulation at 45 CFR
155.220(f)-(g).Termination Reconsidered or Suspension LiftedIf applicable, this is the date the termination
was reconsidered or the suspension lifted. After
this date, the agent or broker may assist with or
facilitate enrollment of qualified individuals,
qualified employers, or qualified employees in
coverage in a manner that constitutes enrollment
through the Marketplaces, or assist individuals
in applying for advance payments of the premium
tax credit and cost-sharing reductions for
qualified health plans (QHPs) offered through the
Marketplaces, under the terms of their reinstated
Marketplace agreement(s).Notes:CCIIO may terminate the FFM registration of an
agent or broker per authority provided in 45 CFR
155.220(g). FFM Registration takes place on the
CMS Enterprise Portal, where agents and brokers
create an FFM User Account, complete identity
proofing, and complete applicable training through
either a CMS-approved third-party vendor or the
